****Please provide current vaccination records 48 hours in advance of your stay or grooming visit. If you make an appointment for grooming or boarding within less than 48 hour please be prepared to provide your records to us. We ask that you contact your veterinarian for us. Many clinics are now requiring owner’s permission to release records. Veterinary clinic copies from owners received by fax or email are also acceptable.****

This is the boarding sheet we use for each visit. Feel free to download and use for all of your stays at Hilltop.
The first two documents are only needed for new clients for Hilltop Dog Hotel:
Grooming Drop-Off Form (for ALL NEW grooming clients and Returning clients to be completed yearly)
We also offer access to all this information on our Gingr customer portal.
Our boarding space is limited, so we ask that you call to make your reservations as soon as possible to ensure we accommodate your dates.
Also, the health and safety of our boarding dogs and staff is very important to us. Below are our requirements for our boarding dogs.
Dogs must have veterinary paperwork showing an exam within the last 12 months.
All guests must be current and up to date on Rabies and Distemper vaccines. Our required vaccines are listed below.
We strongly recommend flea and tick prevention be up to date while your dog is boarding. Our nature walks are in a wooded area where they may be exposed to fleas and ticks.
Vaccine records and veterinary paperwork must be received prior to drop off. Veterinary clinic copies from owners received by fax or email are also acceptable.
Vaccines required for boarding are:
Rabies
Distemper Vaccine
Bordetella or Kennel Cough every 6 months
Canine Influenza Bivalent (H3N8 and H3N2 strains)
Please remember there is a charge of $30.00 for Sunday pickup per client, not per pet. Thanks!
